Sounds like you're printing out of a Microsoft application. This is an option to print the document properties that has been selected. Open the application, go to your apps toolbar and select “Tools,” then select “Options,” now select the “Print” tab, find the section labled “Include with document,” find “Document Properties” and click on the check box to clear it. Now whenever you print you should get just what you printed, not the document properties along with it.

1. Does this happen only in one program? Looks like Microsoft Word, but I just want to confirm.
2. If you open the same document in Wordpad and print it, does this information print out as well?
Trying to narrow down the problem. But my guess is that the “template” for you Word documents (the default one for all documents) has become corrupted.
